Telehealth Informed Consent

Before initiating care through Hypergevity, you must read and acknowledge this Informed Consent. It explains the nature of telehealth, the asynchronous evaluation model, the risks and benefits of remote treatment, and your rights as a patient.

Section 01

Nature of telehealth services

'Telehealth' means the delivery of healthcare using electronic communications, information technology, asynchronous store-and-forward review, or other means between a licensed clinician in one location and a patient in another. Through Hypergevity, an independent U.S.-licensed clinician will review your intake responses and any uploaded documents and may, at their sole clinical discretion, issue a treatment plan, prescription, or referral.

Asynchronous evaluation model

Most evaluations are conducted asynchronously — you submit information at one time, and a clinician reviews it within 24 hours. Synchronous (live) visits may be required for certain therapies. You may be asked to provide additional labs, photos, or vitals before a prescription is issued.

Potential benefits

  • Convenient access to U.S.-licensed clinicians without travel.
  • Faster initial evaluation and follow-up than in-person care models.
  • Continuity of care through secure messaging and patient records.
  • Discreet, cold-chain delivery of prescribed therapies.
Section 04

Potential risks & limitations

  • Information transmitted may be insufficient for proper diagnosis; the clinician may require an in-person evaluation.
  • Technical failures may delay communication, evaluation, or treatment.
  • Security protocols may fail despite reasonable safeguards.
  • The provider may determine that you are not a candidate for treatment.
  • All medications carry risks of side effects, allergic reactions, or interactions.
Section 05

No emergency services

Hypergevity is not designed for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately. Do not use this platform to communicate urgent or life-threatening conditions.

Section 06

Your rights

  • You may withhold or withdraw consent to telehealth at any time without affecting your right to future care.
  • You have access to all medical information transmitted during your evaluation.
  • You may request that an in-person consultation replace a telehealth visit.
  • You will be informed of all parties present during any synchronous encounter.
Section 07

Compounded medications

Where compounded medications are prescribed, they are dispensed by independent U.S.-registered 503A or 503B compounding pharmacies. Compounded medications are not FDA-approved products and may not have undergone the same review as commercially manufactured drugs. Your clinician will discuss the rationale for compounding when applicable.

Acknowledgment

By proceeding with intake on Hypergevity, you confirm that you are at least 18 years old, that you have read and understood this Informed Consent, and that you accept the risks and benefits described above. You may print or save a copy of this document for your records at any time.
